URGENT NOTICE TO BENEFICIARIES OF THE NBA AFFORDABLE HEALTH PLAN.
Dear Sir/Ma,
In furtherance of the NBA AFFORDABLE HEALTH PLAN, aka, AKPATACARES, we are pleased to inform you that the process of onboarding the first 1000 members of the NBA on GIFSHIP (of which you are a beneficiary) has commenced.
Registration Formalities.
Beneficiaries are to comply with the following guidelines:
1. Fill the NBA customised registration form with all the necessary details. Each form is to be completed by each beneficiary. The scheme is the Group, Individual and family social health insurance program (GIFSHIP).
2. All completed registration forms must be scanned and sent to david.ezekiel@nigerianbar.org.ng on or before 25th October, 2021.
3. Any beneficiary who fails to return his/her duly filled registration forms will automatically be replaced.
4. All beneficiaries must provide their SCN and affix their NBA stamps and seals on the registration form.
5. In choosing a hospital, beneficiaries are advised to choose hospitals that are close to their residence and are registered with the NHIS.
6. The following documents are attached herewith:
a. The NHIS enrollment programme.
b. Rights of the enrollees.
c. The NBA Customized Registration Form.
d. The comprehensive coverage of the GIFSHIP.
e. List of hospitals registered with NHIS.
7. The registration forms must be filled in CAPITAL LETTERS.
